Cousins are lifelong friends or some such BS. It was on a needlepoint pillow at a thrift store somewhere.
Honestly though, we've been pretty close most of our lives, being the only children of twin sisters and our dads being only children, we didn't have any other cousins and being born within months of each other meant we did a lot together growing up.
We'd lived in the same city and our parents often double dated. On those nights we got to have sleepovers at one or the other's house while sharing a babysitter. Something we'd each inherited from our moms was bedwetting, well beyond potty training age.
The fact that both of us had the same struggle made for far less fuss when we got our pull-ups out to get ready for bed during those sleepovers.
However, even family can grow apart. We're soon old enough not to need a babysitter for date nights and living on separate ends of a big city, we go to different high schools which means we started to see each other less and less.
More like a few times a year during holidays.
We also ended up at different colleges and due to scheduling and whatnot, we went years without seeing each other. When our parents offered to take us on vacation together to celebrate graduating college the last thing we expected was one of us (or both?) would still be a bedwetter.
So I am thinking in this RP one or both of them have continued to wear protection to bed either out of need or desire.
This can go a few directions depending on each of our interests but is meant to be primarily a diaper lover RP. If you like the idea, but the characters being cousins creeps you out, we can discuss other options maybe we're just long time family friends.

I'm open to this being sexual but it doesn't necessarily have to be. My preference is for partners/ characters who lean feminine.
