LAST PICTURE SHOWS BLOOD

I went to get two stones removed, one urethral and one bladder. The bladder one ended up being huge. I also wanted to get the urethra all the way to the tip as well as getting some sensation to the phallus by connecting some nerves to the new forearm graft.

All of that was done successfully, the surgery took 8.5hours. Mr. Christopher said it was quite difficult to find where they had buried the clitoral nerve but he did find it so he connected that as well as a tactile nerve.

I'm now 5 days post op and I'm leaving the hospital tomorrow. I've stood up twice now and they took me for a shower yesterday, thankfully I've not fainted either time, it's just a rough road to standing up right now. They removed the drains yesterday (day 4) so that helped out a bit with mobility (removal doesn't hurt).

My hand was in a lot of pain at first, it's still tender but not as much. I have to keep it up right like this until Monday where they'll change the dressings to allow me to stretch it flat, that way I can have it flat but elevated on a pillow to prevent swelling. He covered it with a split thickness graft from my thigh, I've yet to see it on my arm but I've seen how it's healing from where they took it (last photo). He said to avoid moving at the wrist until week 2 and do exercises where I tap my fingers.

I'm at the fitzrovia hospital and the care has been really good, Mr. Christopher was also very thorough all throughout this journey. They've already booked all my post op appointments for twice a week for the next 3 weeks. At first you get checked up on every hour- 2 hours and get given painkillers and antibiotics, since yesterday they check up every 4 hours. They take blood pressure, heart rate, temperature and Doppler test on the graft. All good so far, Mr.Christopher said the chances of something happening to the graft now are really low but to keep an eye out if the skin changes colour to black, you can also technically feel the pulse yourself with your fingers if you want to be sure once you've left the hospital.

Not sure what else to add right now, I'll take some more pictures as time goes on and post those and see how my voiding trial goes in about 2 weeks time.

Mr. Christopher Nim up close consultation

I recently travelled to London to get seen by Nim to get my arms and phallus checked up close.

I was told everything seems okay but he'll be taking the skin from the tattooed arm as the other one has some scars and it would add some risk. He said he's going to try and get two nerves to hookup.

We discussed a different stage, minimum 3 months after this one, for glans and getting my scrotum revised from bifid to VY, the quote for that privately was 13k.. my date is on the 3rd of August and I'm very excited to finally get this done.

He estimated a 60-90% chance of getting sensation depending on the amount of nerves harvested and he's going to try to get access to my cl**toral nerve for erotic feeling.

I'm attaching a video of the amount of skin he'll be taking from my arm, he drew this on based on my phallus length plus a bit more to be safe for length and the width seemed kinda standard I think.

Dr. Julia Bohr consultation

Hey y'all, anyone following my journey knows I'm between two surgeons for my urethral reconstruction. I had my consultation with Dr. Bohr today and she was quite the opposite to Nim. She recommended a two stage skin graft repair but instead of a split skin graft, a full thickness skin graft to prevent shrinking. She said it wouldn't be possible to get a graft from the forearm to gain sensation because it's just not feasible. I don't have a quote yet but I'll include it in an edit when I do. I've decided to go with Nim and try my luck with his method.

Question for RFF urethra

Although a different process I might be getting a new urethra done from my arm and also introduce some sensory nerves to my phallus. I was told today by a different surgeon that urethras made by normal skin and not grafts that are made for wet environments like the cheek aren't a good option for urethras short term or long term. Can people that have gotten UL with an arm flap tell me what their experience has been like since getting it done.

Consultation update Dr. Nim Christopher

For those who don't know I've had an MLD flap with UL and had fistulas and strictures throughout. I found Dr. Christopher with the help of people from this forum.

I am very happy to say that he's gonna be attempting to use a forearm draft, nerve hookup included for a new urethra and also possibly develop some sensation on my phallus, something I have very little of now due to my donor site. He will also consequently fix where my meatus is as currently it's under my glans like hypospadias. I'm gonna be paying out of pocket so I'm waiting for a quote and hopefully I can get this done soon-ish.
