I have a stricture and ended up having a catheter placed. Unfortunately I won't be 6 months post op until October. By then it'll be the holiday season and I doubt I will be able to get urethralplasty until after the holidays. I will probably have this catheter for the remainder of the year. It barely been 10 days since having the catheter and the spasms are killing me. I drink a lot of water, I take 2 antispasm medications and also stool softeners. I tried using a valve but honestly it just stresses me out since I can't tell if my bladder is full or if its just a spasm. I remember having a suprapubic catheter my 1st month post op but I don't remember the pain being this bad. Sometimes I honestly can't sit, stand, or even lay down. Nothing I do helps me be comfortable. My mom told me that the first 2 weeks I was on pain killers and thats probably why I didn't even get bothered by the catheter.
Please tell me it gets better. Do the spasms calm down after the 1st month?

I'm currently looking into my options and was wondering if anyone here has undergone a true one-stage phalloplasty (specifically combining hysterectomy, vaginectomy/colpectomy, urethroplasty, and phalloplasty all in one single surgery).
If you feel comfortable sharing, I’d really appreciate hearing about your personal experience. Specifically, I'm curious about a few things:
- How did the surgery and the overall recovery process go for you, considering how extensive it is?
- How are you feeling with the results?
- Knowing what you know now, would you choose to do it all in one stage again?
Any insights, advice, or shared experiences would be incredibly helpful.

I’m 2.5 years post op RFF phallo. Unfortunately I dealt with a lot of post op pain related to a stricture and also had a catheter for a year so really only in the last 6 months or so has my dick gone from a medical wound site to a penis. I didn’t do any nerve rehab due to the depression from mental trauma and am pretty unsatisfied with my sensation. I have had medical tattooing so I know the nerves are working but I don’t feel erotic sensation on my dick at all. I can get off easily but I feel like it’s because my balls brush against my buried tdick when I tug and that’s where the orgasm comes from. Is it like this for everyone? Are my expectations too high?
I’ve recently gotten into a relationship with another trans guy and I don’t have an ED yet but we do like frotting and whatnot. I’m wondering if anyone else can feel that? Like yes objectively it’s hot and that turns me on but I can’t FEEL it the way he can. Not at all really. I’m hoping with ED soon the pressure of being inside someone will be pleasurable but who knows.
Final question, has anyone had any success doing the OSHU nerve rehab guide years after initial nerve hookup? Just looking for either some encouragement or a call to accept things as they are if it’s time for that.
Looking to hear only from post op guys please regarding sensation but welcome any encouragement or solidarity from anyone!

Hoping to make the most of my next few months of no surgery before upsizing my balls and a final mons adjustment to hopefully wrap up my phallo process by the turn of the year. I signed up for some workout classes through my local community college for free at the advice of others in this thread after realizing many of the community colleges around here are at least fall free if you’ve been a resident a year. Would love to connect with other guys who are in the journey and trying to keep our bodies moving in the process; I’m primarily a jogger or yoga kind of person, but signed up for a swim class, basketball & ju jitsu for the fall term and am open to other activities to try as a group. LAST PICTURE SHOWS BLOOD
I went to get two stones removed, one urethral and one bladder. The bladder one ended up being huge. I also wanted to get the urethra all the way to the tip as well as getting some sensation to the phallus by connecting some nerves to the new forearm graft.
All of that was done successfully, the surgery took 8.5hours. Mr. Christopher said it was quite difficult to find where they had buried the clitoral nerve but he did find it so he connected that as well as a tactile nerve.
I'm now 5 days post op and I'm leaving the hospital tomorrow. I've stood up twice now and they took me for a shower yesterday, thankfully I've not fainted either time, it's just a rough road to standing up right now. They removed the drains yesterday (day 4) so that helped out a bit with mobility (removal doesn't hurt).
My hand was in a lot of pain at first, it's still tender but not as much. I have to keep it up right like this until Monday where they'll change the dressings to allow me to stretch it flat, that way I can have it flat but elevated on a pillow to prevent swelling. He covered it with a split thickness graft from my thigh, I've yet to see it on my arm but I've seen how it's healing from where they took it (last photo). He said to avoid moving at the wrist until week 2 and do exercises where I tap my fingers.
I'm at the fitzrovia hospital and the care has been really good, Mr. Christopher was also very thorough all throughout this journey. They've already booked all my post op appointments for twice a week for the next 3 weeks. At first you get checked up on every hour- 2 hours and get given painkillers and antibiotics, since yesterday they check up every 4 hours. They take blood pressure, heart rate, temperature and Doppler test on the graft. All good so far, Mr.Christopher said the chances of something happening to the graft now are really low but to keep an eye out if the skin changes colour to black, you can also technically feel the pulse yourself with your fingers if you want to be sure once you've left the hospital.
Not sure what else to add right now, I'll take some more pictures as time goes on and post those and see how my voiding trial goes in about 2 weeks time.
I have only had one appointment. He seems very down to earth and knowledgeable, lgbtq+ affirming. Lots of reasons for starting therapy I don’t really want to dive into, but as I’m sure some of you might be able to understand I am a hot mess of sexual/medical trauma since before I even had surgery so like. I’ve got shit to work out.
Anyway he gave me a handout on mindful self touch and I tried it yesterday. 20 minute timer. Told myself to write down my thoughts/feelings/sensations as I go because I have the memory of an ADHD goldfish and am prone to dissociating, so I wanted to make sure to capture it.
Without getting triggery. Suffice to say I think I may have some internalized transphobia to work through. Only lasted 10 minutes before I had to stop to protect my mental health.
Last stage coming up in September (vnec and rod/replacement ball for the one I lost to infection). locked out of UL at this point but that was never in the cards for me.

I know this has been asked before, I know this is a common issue, I just cannot wrap my head around how to do this. The skin graft is explainable to new people. But I have to tell my coworkers I’m going on medical leave. Then another medical leave. Then another medical leave. My manager would not be chill about it if he knew I was trans. I can’t just get a desk job, I need to continue working at this store for health insurance.
Edit: I am friends with most of my coworkers and we all know each other very well.
Here is an update regarding the size reduction of the phalloplasty...(MLD)
Before (Picture 1), it measured 27/28 cm in length with a circumference of 23 cm.
Now (picture 2) , it is 25 cm long and 16 cm in circumference... I feel like my surgeon reduced the thickness too much; 17 or even 18 cm of circumference would have been preferable.
For the moment, it looks thick in the mirror, but in reality, I find it quite thin... As for the length, 25 cm suits me perfectly, and my surgeon does not wish to alter it.
I woke up at 5am to pee and felt wetness in my boxers. Tons of yellow/clear thick drainage. On the way to work even more this time more yellow green. It’s been ALL DAY. Not just a little. Enough to soak paper towels through. I changed them out 5 times today. I reached out and haven’t heard back yet
was anyone else still struggling to walk normal/fast at 2 months post op? I’ve made progress in the past month or so but I still struggle to walk fluidly with straighter legs. I start university in late Sept. and I’m worried I won’t be able to walk around a huge campus with how things are going.
i included a picture to show what i’m talking about but when i was discharged from the hospital i was cleared by dr freet to stop wearing the donut and instantly started to develop a lot of wound separation. that was 3 days ago now and im back to the donut. did anyone else have this happen and why? picture of my phallus included to show what I mean
So I’ve noticed this little red line along the bottom of my penis for maybe 2 weeks now? Towards the base, across the scar line. At first I thought maybe I rubbed it wrong and didn’t feel it since I don’t have sensation there that much yet, but it hasn’t gone away. Maybe it’s just from the way it lays naturally? The skin isn’t used to it, after all. Maybe it just needs a minute to adjust? Does anyone else have experience with this?
I’ve got a pretty strong stream with my natal anatomy. When I pee standing with my natal anatomy, I can typically reach a distance of about 2-3 feet without any effort. (Problem is, I can’t pee standing with my natal anatomy with pants on… so pretty useless outside the shower and my own bathroom).
After you had your phalloplasty, did you notice any difference in your stream as far as strength or velocity goes?

I quite honestly may delete this post at some point because I don’t especially enjoy the idea of posting dick pics on public forums, regardless of whether any identifying features are shown, but for the time being I will share the visuals for anybody interested. If anybody is curious about the leg scar you may ask via direct message, I have very identifiable tattoos on my legs which I am not comfortable posting publicly.
IN HOSPITAL
Dr Gurjala informed me between surgery days that flap bloodflow was not looking good. Surgery notes describe perforator entering capsule rather than flap and other anatomical obstacles that contributed to this issue. UL was scrapped as a result which I was ok with, but he said he was worried about flap surviving at all. He made clear the potential outcomes [successful phallus creation, phallus creation that does not survive after a few days, no phallus creation] and that he would do his best to make things work.
Phallus creation was successful, but as a result of the poor bloodflow, pressure had built up in the phallus as the veins were not robust enough to return blood to the body. Lower seam of the phallus was left open to alleviate some of the pressure. Leech therapy was used for 72 hours to relieve pressure as well, which gave the weaker blood vessels an opportunity to strengthen enough to support the phallus themselves, which they did. I received 2 blood transfusions from the blood loss during that time. After leech therapy concluded, circulation has been great ever since.
Recovery in the hospital following that was very smooth and easy. I was able to walk fairly easily after slowly working up to it over the course of a few days. PT discharged me with a cane, which I used intermittently for about a week. The catheter that was placed was removed before I was discharged.
RETURN HOME
I had a small scare with excessive swelling in my hip/groin between the single scar and phallus when I first arrived home (I suspect from the exertion of taking the stairs to my apartment) which resolved within a few hours after icing. The first week was mostly sitting around, walking around my home for a little bit, sleeping, watching tv… my leg felt very irritated due to the external tissue expander (seems close) and the drain in my leg, which had been yanked on accidentally in the hospital and the stitch holding it in place was easily aggravated. Phallus wasn’t painful, used cloud dressings to prop it up, aquaphor or wet/dry dressings on leg and phallus on the unhealed parts.
Dermaclose and drain were removed a week after hospital discharge. The staples being removed greatly increased my ability to move around, and within a couple days I was up and about running errands and hanging out with friends. Range of motion has not completely returned in my donor leg, I feel tightness when I lift my knees to my chest and the incision has not completely healed so I am not rushing to stretch too far. Donor leg is also slightly weaker than other leg, I can handle stairs just fine but I can feel a difference in strength. There is no pain in my leg after staple removal. Wound healing on leg was extremely itchy after that, I slathered it in aquaphor which relieved the sensation somewhat.
Stitches on the phallus were removed today. They had also been itchy, and I had been using aquaphor on those as well. Itchiness is gone. I was told I could start dangling pretty early on, and haven’t been using the cloud dressings to prop for at least a few days now. I normally just lean the phallus against my leg and haven’t had any issues with that. I forgot to bring the questions I had written down so I was unable to remember them for this appointment, but I am planning to email them to my favorite nurse in the clinic to ask if she can get the questions answered and/or ask Dr Gurjala directly.
CURRENT SURGICAL OUTCOME/SATISFACTION WITH PHYSICAL ASPECT
I am beyond happy with how the phallus looks. I’m pleased with the size (although I am preparing for some shrinkage as swelling continues to go down, so I am trying not to get attached to those measurements) and despite the leech bite marks, I think it looks great. I felt a very strong connection to it instantly. I am sure this is entirely a mental thing, but when I touch it I can feel a dulled sensation pretty much everywhere, and can feel pressure throughout. I occasionally feel random buzzing or pinching feeling in the base as well, without physical stimulation. However, somebody else was touching me and I had my eyes closed, I highly doubt I would feel much of anything at all. It is just my brain telling me I /should/ feel those sensations when I am touching it. Which is still awesome, the visual of it and the mental map of my body matching what is physically there feels so relieving. Feeling the weight of it attached to my body feels… comforting? I struggle to find a word that expresses what I am trying to convey. It feels great.
The lack of glans and scrotum/testicles at this point and my dysphoria regarding those is for the most part overshadowed by the current relief I feel about my penis. I do dislike that the foreskin is still below the phallus, but I know it is going to be used in my next stage. And I cannot really see it unless I am actively trying to.
I had a very rocky road to get to this point in terms of recovery and complications during tissue expander, abandoning UL last minute, and the concern about flap loss in the first few days post-op, but it was all so worth it. I am so happy with how things have ended up and I hope to continue to be this pleased as I continue recovering.
I got glansplasty, we centered my penis because it was tugging to the left, added my left testicle,had plication done and did the 2nd stage of the Johansen (hot dog) urethral repair 😌

I’m very pleased with how my penis looks. Only one more surgery to go! And that’s for my erectile device. Ask any questions you may have 🙂

I posted about a year ago* with my then new guiche and two hafada piercings. One of the hafadas rejected, but the others healed beautifully. I got the third redone about three months ago, and it seems a lot happier this time around.
Today I've added a second guiche (the higher one) and a reverse PA and I'm delighted with the results. Piercer today was Marcus, last year's was Helen, both at Holier Than Thou in Manchester. Phallo was done by the London Team, with stage 3 completed about four years ago.
Got a letter from my insurance today saying that my Phalloplasty has been approved, yay 🎉Approved from 8/30/26-9/01/26, that’s 3 whole days, errrg. I told my “contact person” that I still have months of electrolysis to do. I didn’t expect it was going to be approved for 3 weeks from now. Plus I still have to arrange for caregiving with T4T, they say not to contact them until you have a surgery date. Do doctors listen to what you tell them? Sorry for the rant ☹️
hey i had rff with freet the first time on june 9th, but only the right side of my dick survived. I then had rff again (but only a portion of it) for my left side of my dick july 29th. i’m home now and im having really bad wound separation on the underside of my shaft. i think it’s just from swelling. it what should i do? i already contacted my surgical team im just wanting to hear from yall
Persistence pays off. I have been fighting, but i finally got my approval letter. Dont let let those insurance fucks tell you have to go to Align. (They bullied me and now im up shits creek without a paddle) Fight them and go to Dr Chen if thats who you believe in.
Youll most likely recieve a call saying they arent in network and this is who is. Tell them that that person doesnt have the experience thats required of a surgery this detailed. And that you dont feel safe or comfortable with them. That youve read accounts of past mistakes and that Dr Chen has the experience needed and you will cont to appeal if need be.
