I never in my life received this pain a pain in my entire first time Anal sex caused pain tailbone gooch pain lower back pain that radiates down legs I took a picture and saw this

Medical Summary & Timeline

  1. Incident & Trigger
    Initial Encounter: First Time Anal Sex November 2024.
    Lasted for a few days it healed with ice then it went away I thought I was fine all I had was these brief lil spasm that a shoot up inside the anus it’ll happen and Randomly and last
    Like 2 seconds then 10 months later the pain came back after I was heavy lifting
    Pain Trigger: Heavy lifting (weightlifting/physical strain) occurring prior to September 2025 testing.
    Current Symptoms: Lower back and tailbone pain with radiating sensations (tingling/numbness) traveling down the legs. And perenium pain
  2. Lab Testing (September 2025)
    Tests performed approximately 10 months post-exposure, well outside all diagnostic window periods.
    STI Screen (Negative/Non-Reactive):
    HIV 1/2: Negative
    Syphilis (RPR): Negative
    Gonorrhea & Chlamydia: Negative (tested via blood, urine, and rectal swab)
    Hepatitis B & C: Negative
    HSV-2 (Genital Herpes): Negative
    HSV-1: Reactive (Positive). I was positive for hsv1 October 08 2024 and never had anal sex that day or anytime before that Nov 2024 was the first time ever and days after is when I first ever had the anal pain Note: This is indicative of common oral herpes (cold sores) and is not typically associated with back/leg nerve pain.
  3. General Health & Urinalysis Findings
    Urinalysis:
    WBC Esterase: Trace Abnormal (but No Bacteria seen on microscopic exam).
    Ketones: 3+ (High).
    Blood Chemistry:
    Glucose: 65 (Slightly Low).
    MCH: 26.0 (Slightly Low).
    Total WBC Count: 4.6 (Normal range, but a decrease from previous baseline of 6.3).

Went to a surgeon he ruled out fissure and hemmeorids and herpes

Had a chat with a pelvic floor therapist they ruled out herpes as well and even doctors

I just don’t understand what could be going on
